Community physiotherapy services (south east Essex)

Our Community Physiotherapy team provides expert support to help you to stay as independent and mobile as possible.

This can be done in the comfort of your own home or through our local neuro-outpatient service.

Who we’re not able to support

Unfortunately, we are unable to provide:

  • intensive rehabilitation (you may need a reablement referral instead)
  • personal training or fitness support
  • therapy if you are already receiving rehab from another NHS or community service.

Home visits for housebound patients

If you're unable to leave your home, our team can visit you to offer personalised advice and guidance. We support you with:

  • regaining or maintaining mobility.
  • building confidence with everyday tasks.
  • reducing your risk of falls.
  • helping you to live independently.

Neuro-outpatient service at Southend Hospital

For adults aged 18 years and older that live in the Southend, Castlepoint and Rochford areas we offer specialist physiotherapy for those with neurological conditions.

This includes help with conditions like stroke, Parkinson’s disease, multiple sclerosis, and more.

We help patients who:

  • need help returning home from hospital.
  • are struggling with reduced mobility.
  • are recovering from an illness or injury.
  • need help managing a long-term or life-limiting condition.
  • are unable to travel to a clinic for treatment.
  • need mobility assessment and support in their own home.

Referrals

Only healthcare professionals can refer to us. We do not accept self-referrals.

  • GPs - refer via MSK- CATS (S1).
  • Other healthcare professionals - email mse.community.physio@nhs.net with the patient’s full details (name, DOB, NHS number, address and reason for referral).
